- Medical record numbers
- Any detailed health information
DO NOT Include in Any Form Submission
Please do NOT include the following in any form submission:
- Your specific medical conditions beyond general categories
- Medication names or dosages
- Doctor's notes or medical records
- Insurance policy numbers
- Any information you would not want publicly disclosed
If You Are a Patient
Selecting a general condition category (such as "Glaucoma" or "Dry Eye") from our dropdown menu is used only to route your inquiry appropriately. This general category selection, combined only with basic contact information, does not constitute PHI.
If you are interested in participating in a clinical trial:
- Submit an inquiry through our Patient form with basic contact information only
- We may contact you about potential opportunities
- Any clinical trial screening will occur through proper clinical channels with appropriate informed consent and HIPAA authorization
For Clinical Trial Participants
If you become a participant in a clinical trial conducted at an ARI network site, your PHI will be handled according to:
- The clinical trial's informed consent form
- A separate HIPAA authorization form
- All applicable federal and state privacy regulations
These documents will be provided to you as part of the informed consent process, which is separate from any website interaction.
